All services require an in-person health consultation prior to scheduling. This protects patient safety and allows individualized care planning. No direct service appointments can be booked until after the consultation.

  • To ensure the highest level of clinical safety and sustained recovery, our packages are delivered in 4 to 6-hour sessions, unless otherwise noted.

Vital Pulse | Hourly Support

  • Focused, professional nursing care designed for flexibility. Ideal for same-day recovery, post-procedure assistance, or new diagnosis support where you need immediate bedside education to get settled.

  • 2-hour minimum required.

Vital Mend | Specialized Wound Care

  • ​Expert in-home clinical care for post-operative recovery, surgical incisions, or chronic wounds. We bring wound management and chronic illness monitoring to the comfort of your home.

  • Pay-per-visit model.

Vital Respite | Caregiver Support

  • Designed specifically for full-time caregivers. This includes a senior wellness check and family liasion services. Use this time to rest, run errands, or attend to personal matters while we provide your loved one with expert clinical oversight and a detailed status update.

  • Includes 4 hours of nursing care.

Vital Conncection | Senior Advocacy

  • Expert clinical oversight for families managing an aging parent’s care from near or afar. We bridge the connection by managing the high-level coordination, including Provider Liaison Services and Clinical Wellness Assessments, so you can focus on being a daughter or son again, not a case manager.

  • Includes weekly, bi-weekly, or monthly 2-hour visits.

Vital Transition | Home Integration

  • Our premier "Bridge" package. Designed for those moving from a hospital or rehab facility back to the home. We manage the logistics of Medication Management, safety assessments, and patient education to ensure you are stable and confident in your new environment.

  • Includes 6 hours of nursing care.

Vital Anchor | Comprehenisive Stabilization

  • We manage the high-level details, including New Diagnosis Support and intensive symptom monitoring, so you can focus entirely on acclimating to your space and healing.

  • Includes 8 hours of nursing care.

Vital Horizon | Extended Clinical Care

  • Seamless, nurse-led support for total continuity. From Chronic Illness Monitoring to complex medication regimens, we oversee every clinical touchpoint of your day to prevent readmission.

  • Includes 12 hours of nursing care.

Vital Infinity | Continuous Oversight

  • Our most comprehensive package for those requiring ongoing, long-term clinical monitoring. This package ensures a nurse is in your home daily - up to 6 days, to manage Chronic Illness, act as a Senior Wellness Liaison, and provide expert advocacy.

  • Includes 24 hours of nursing care.
